Resumen de Malformaciones del sistema nervioso central anencefalia y encefalocele: Reporte de dos casos

Se presenta un caso de anencefalia y otro de encefalocele; defectos de cierre del tubo neural fenotípicamente distintos, con diferente pronóstico y distinto manejo terapéutico, atendidos en el Hospital Materno Infantil Germán Urquidi de Cochabamba.
El 1er caso corresponde a una primigesta de 19 años que acude con trabajo de parto de 8 horas, con gestación de 23 semanas; sin ningún control prenatal; con signos vitales estables, altura uterina 19 cm, producto en situación indiferente DU de 3/10/30/++, FCF negativa;
cérvix central con 4 cm de dilatación, membranas integras, polo pélvico. Ingresa con diagnósticos de: G1P0, embarazo de 23 semanas, óbito fetal, trabajo de parto; atendiéndose parto de óbito fetal de sexo femenino con peso de 985 gr, APGAR 0; con ausencia de encéfalo y calota craneana, exoftalmos y espina bífida.
El 2º caso corresponde a una segundigesta de 21 años referida del Hospital de Vinto, asintomática con altura uterina 20 cm, producto en situación longitudinal, presentación pélvica, FCF de 140 x min, sin dinámica uterina ni modificaciones cervicales. Ingresa con diagnósticos de: G2P1, embarazo de 25 semanas, feto vivo, malformación congénita. Los laboratorios reportan: Infección urinaria, toxoplasmosis, citomegalovirus y alfa-fetoproteína elevada;
ecografía compatible con acrania y cefalocele occipital. Se induce parto obteniéndose óbito masculino con peso de 1086 gr, talla 37 cm, APGAR 0, con solución de continuidad en región occipital por el cual protruye el encéfalo con sus meninges y cuero cabelludo.
Ambos pacientes cursan su puerperio favorablemente.

It is reported one case of anencephaly and one encephalocele. They are defects of neural tube phenotypically different, with different prognosis and different therapeutic approach, they were at Hospital Materno Infantil Germán Urquidi of Cochabamba.
The first case is a 19 year old primigravida who comes with labor for 8 hours, with gestation of 23 weeks, without any antenatal care, with stable vital signs, uterine height 19 cm, product indifferent situation UD 3/10/30/++, fetal heart rate inaudible, 4 cm of central cervical dilatation, intact membranes, pelvic pole. She is admitted with diagnoses of: G1D0, 23 weeks pregnancy, stillbirth, labor, it was attended a stillbirth delivery of female sex weighing 985 grams, APGAR 0, with no brain nor cranial, exophthalmos and spina bifida.
The second case is a 21-year second pregnancy woman remitted to Vinto Hospital she was asymptomatic with uterine height of 20 cm, fetus in a longitudinal, pelvic presentation, fetal heart rate of 140 x min without dynamic uterine or cervical changes. She was admitted with diagnoses of: G2 D1, 25 weeks pregnancy, live fetus and congenital malformation. The laboratories report: Urinary tract infection, toxoplasmosis, cytomegalovirus and high alphafetoprotein.
Ultrasound matches occipital cephalocele and acrania.
Delivery is induced obtaining a male death product with a weight of 1086 g, height of 37 cm, APGAR 0, with solution of continuity in the occipital region in which the brain protrudes with its meninges, and scalp.
Both patients enrolled in the puerperium favorably.
