Breast cancer is associated with aging. The majority of breast cancers are diagnosed as early stage (nonmetastatic), and are potentially curable. This article reviews treatment of early stage breast cancer in older adults, with a focus on the risks and benefits of surgery, radiation therapy, endocrine therapy, chemotherapy, and trastuzumab. Data regarding treatment are limited at the extremes of age and in patients with significant comorbidity or functional decline. Also included is a review of toxicities (both short- and long-term) from common cancer therapies in early breast cancer. Secondary screening and prevention weighed against life expectancy are discussed.
