While use of multiple medications or polypharmacy might generate positive outcomes for some older adults with multiple diseases, there is increasing evidence that it is associated with increased risks of adverse events. Therefore, rational withdrawal of medications may be the appropriate clinical decision and may result in significant clinical and functional benefits in some older people exposed to polypharmacy. This clinical review aims to highlight the evidence in older adults for the effectiveness of various types of interventions used to reduce polypharmacy, and the impact on prescribing and clinical outcomes.
