Diferenciación clínica entre la depresión mayor y la depresión en el trastorno afectivo mayor

    La tristeza patológica, o la disminución del estado de ánimo es uno de los motivos de consulta más frecuentes en los diferentes niveles de complejidad de los servicios hospitalarios colombianos, ya sea como parte del trastorno depresivo mayor o dentro del trastorno afectivo bipolar. A pesar de que se considera que la forma de presentación de los cuadros depresivos, es similar dentro de ambos trastornos, es posible encontrar algunas diferencias clínicas que pueden ayudar al médico a pensar en una de las dos posibilidades. El objetivo del presente artículo consiste en brindar herramientas útiles tanto para los médicos generales, como para los médicos especialistas no psiquiatras, al momento de evaluar un paciente con un cuadro depresivo, orientando el diagnóstico hacia uno de los dos trastornos. Al realizar un diagnóstico correcto de depresión bipolar, será posible definir las pautas de tratamiento más apropiadas, evitando posibles complicaciones inherentes al tratamiento antidepresivo.

    The pathological sadness, or the diminution of the mood is one of the more frequent reasons for consultation in the different levels from complexity of the Colombian hospital services, or like part of the greater depressive upheaval or within the bipolar affective upheaval. Although it is considered that the form of presentation of the depressive pictures, is similar within both upheavals, is possible to find some differences clinical that they can help the doctor to think about one of the two possibilities.

    The objective of the present article consists of as much offering useful tools for the general doctors, like for the specialist doctors nonpsychiatrists, at the time of evaluating a patient with a depressive picture, orienting the diagnosis towards one of both upheavals. When making a correct diagnosis of bipolar depression, will be possible to define the more appropriate guidelines of treatment, avoiding possible inherent complications to the antidepressing treatment.
