Objective: To increase the efficiency of pharmaceutical interventions on drug interactions by selecting these according to the severity criteria and level of evidence. Method: This study was performed at a hospital referral area. To identify and select the interactions, we used: the BOT software application, the Hansten and Horn criteria and the Stockley database. We selected those interactions which met the selection criteria proposed by a committee of experts and classified according to the actions to be carried out. Results: We observed 210 interactions affecting included drugs in the hospital. Of these 210 interactions, 135 were described in at least two databases and 94 were considered serious. Regarding the monitoring of the interactions, 32 showed clear recommendation for the physician and 62 were classified for clinical or analytical monitoring. Conclusions: We Increased the precision of the interventions, reducing the involved interactions to more than the 50%, besides of providing clear and concise recommendations avoiding waste of time for the prescriber
