Resumen de Sexualidad, fertilidad y anticoncepción en adolescentes epilépticos: estado del arte y propuestas de conducta

OBJETIVO: 1) Presentar el estado actual de los conocimientos que pueden dar respuesta a muchas de las preocupaciones de los adolescentes epilépticos, familiares e incluso de profesionales de la salud relacionados con su atención. 2) Hacer recomendaciones sobre las opciones actuales de conducta y tratamiento de estos pacientes, en Cuba.DESARROLLO: Algunos trastornos e incertidumbres concernientes a la sexualidad, fertilidad y la manera de evitar el embarazo en la adolescencia, sobre todo en pacientes con enfermedades crónicas y entre ellas la epilepsia, aún no tienen respuestas y soluciones definitivas aceptadas de manera unánime. En los adolescentes epilépticos hay factores con influencia conocida en la mayor frecuencia de presentación de disfunciones sexuales, infertilidad y dificultades en determinar la combinación del método anticonceptivo y la droga antiepiléptica más adecuada en aquellos sexualmente activos. Estos son: la propia epilepsia y los ataques, la medicación, la asociación con otras enfermedades del sistema nervioso, los trastornos mentales, la ansiedad, la depresión y las características sociales entre otros. Hay diferentes opciones de tratamiento en las diversas situaciones, pero se deben conocer los efectos adversos de las drogas antiepilépticas en la función sexual y reproductiva antes de ser utilizadas.CONCLUSIONES: En los adolescentes epilépticos, en los que influyen tantos factores, es importante tener una “guía” general de conducta ante situaciones especiales, pero finalmente las decisiones deben ser individualizadas.

OBJECTIVES: 1) To present the actual state of the knowledge that may answer a lot of the preoccupations of the epileptic adolescents, relatives and health professionals with dedication to their attention. 2) To recommend about different options of management and therapy of these patients, at present in Cuba.DEVELOPMENT: Some disorders and uncertainties about the sexuality, fertility and the mode to avoid the pregnancy in the adolescence, principally in patients with chronic disorders and among them the epilepsy, do not have, the answers and/or definitive solutions approved by unanimity. In adolescents with epilepsy, there are some factors with a known influence of major frequency of sexual dysfunctions, infertility and difficulty in the determination of the best combination of methods of contraception and the antiepileptic drugs, in the adolescents with sexual activity. These are: the own epilepsy and the attacks, the medication, the association of diseases of the nervous system, the mental disorders, the anxiety, depression, the social characteristics and others. There are different options of therapy in the unlike situations, but should be known, the adverse effects of the antiepileptic drugs in the sexual function and the reproduction before their utilization.CONCLUSIONS: In adolescents with epilepsy, with the influence of many factors, is very important to have a general mode of management in special situations, but the decisions at the end should be personalized.
