Dificultad diagnóstica y terapéutica en un trastorno psicótico de inicio tardío. Reporte de un caso

    Reporte del caso de una paciente que presentó su primer episodio psicótico a la edad de 48 años con predominio de síntomas delirantes, con una grave y tórpida disfunción familiar y social. Ameritó hospitalización psiquiátrica durante 83 días debido a su difícil evolución y múltiples tratamientos farmacológicos que, además, interfirieron en el diagnóstico y pronóstico. Se subraya la importancia del abordaje y del diagnóstico diferencial que deben ser tomados en cuenta por la variedad de síntomas psicóticos que son parte del diagnóstico de esquizofrenia, pero que sin duda no son patognomónicos de ésta.

    This is the report of a case of a patient who presented her first psychotic episode at the age of 48 years old with a predominance of delusional symptoms but with a torpid and severe family and social dysfunction meriting psychiatric hospitalization for 83 days because of her difficult treatment and a considerable multiple pharmacological schemes that interfere on the diagnosis and prognosis; the importance of the approach and the differential diagnosis that has to be taken into account to the variety of psychotic symptoms that are part of the diagnosis of schizophrenia, but they certainly are not pathognomonic of it.
