La fiebre sin evidencia de focalidad en el paciente ingresado supone un reto diagnóstico de primer nivel. A pesar de ser un cuadro muy frecuente en la práctica clínica, no existen muchas publicaciones científicas al respecto, probablemente por la falta de una definición unívoca del término. Es importante diagnosticar cuadros potencialmente graves como los que se asocian a ciertas infecciones. Se requiere una meticulosa anamnesis y una exploración física para orientar el diagnóstico y solicitar las pruebas diagnósticas más adecuadas.

Fever without evidence of focal in inpatient diagnostic challenge is a first level. Despite being a very common condition in clinical practice, there are not many scientific publications about it, probably by the lack of an unambiguous definition of the term. It is important to diagnose potentially serious conditions such as those associated with certain infections. A thorough history and physical examination are required to guide the diagnosis and apply the most appropriate diagnostic tests.
