Review of contraindicated drug-drug interactions of antiretrovirals, in a tertiary hospital

    • The interactions between drugs are possible whenever a person takes two or more medications concurrently. There are a number of mechanisms by which drugs interact with each other; and most of them can be divided into two main categories: pharmacokinetic and pharmacodynamic interactions.

      The aim of this study was to conduct a literature review of clinically relevant drug-drug interactions of antiretrovirals, to build a list of clinically relevant drug-drug interactions. The list will contain the medicines that interact (classified as «contraindicated»), the mechanism of the interaction and the effect involved.

      A systematic literature search was conducted on PubMed using free text terms, reference lists from studies and reviews identified were searched manually for relevant studies.

      Based on the literature obtained above, a list of clinically relevant interactions was built. The list was then double-checked with other databases such as Micromedex ® and the on-line application CheckTheMeds®.

      In this paper, we developed a list of 281 drug interactions that were categorized as major drug-drug interactions and contraindicated drug-drug interactions of antiretrovirals that are frequently found in hospitals. Due to the high number of localized interactions, we restricted the table to consider only the contraindicated interactions to prevent the phenomenon called «alert fatigue». Finally a table with 110 drug interactions of antiretrovirals classified as contraindicated for use in hospitals and electronic prescribing programs was developed
