Centralization of the development of infliximab: Patient involvement and economic analysis

  • Objective: To describe the strategy taken for the centralization of the preparations of Infliximab􀀞 and to perform a descriptive study of costs and the prescription profile.

    Method: It was created a new system of work involving direct patient collaboration in their treatment. The existing Standardized Working Protocols 􀀋(SWP􀀌) were reviewed and new ones were created. An analysis of costs prior the centralization of preparations in the Pharmacy Department was performed to determine the savings that centralization would suppose􀀞 and these results were confirmed by a second analysis determining real preparations were centralized.

    Results: A new working system implemented􀀞 preparation of the mixtures was scheduled evening administration. An answering machine was installed the Pharmacy. The patients used it to request the treatment preparation.

    99% made request in advance as stipulated. In the study conducted prior centralization,134 patients were analysed. The total cost of the treatment with Infliximab,making the necessary preparations in the Medical Day Hospital (MDH) was 􀀌 of 339,515.45 euros;􀀞 if making preparations in the Pharmacy Department,it would have been 307,969.92 euros. After centralization, 158 patients were analysed. The total cost, making the preparations in the Pharmacy Department, was of 372,623.43 euros􀀞 preparing in MDH would have been 403,282.56 euros. Over 50% of patients were treated for Crohn’s disease.

    Conclusion: Centralized preparation of infliximab is able to reduce the costs associated with the drug. The involvement of patients in their treatment has allowed us this centralization, decreasing their length of stay and facilitating work in the Pharmacy Department
