Los procesos patológicos que afectan a las estructuras del periodonto pueden tener diversos orígenes y cursar con una sintomatología parecida. No hay signos ni síntomas patognomónicos que nos indiquen una etiología concreta. Por tanto, es preciso utilizar todas las pruebas diagnósticas disponibles para descubrir el origen de la afectación, e instaurar el tratamiento apropiado. Sólo así se evitarán fracasos diagnósticos y terapéuticos.
En este trabajo se revisan las comunicaciones existentes entre la pulpa y el periodonto, la patogenia de las lesiones endoperiodontales, su pronóstico, su tratamiento y se hace especial hincapié en el diagnóstico y diagnóstico diferencial de dichas lesiones.

The pathological processes that affect the structures of the periodontum may have different causes but present with a similar symptoms. There are not patognomonic signs that tell us the specific etiology. There fore, it is important to use all the avaible diagnostic tests in order to find out the specific origin and thus institute the apropiate therapy. Only in this way, the diagnostic and therapeutic failures can be avoided. In this publication, the different conections between the pulp and periodontal ligament are reviewed as well as the pathogenic processes involved in endo-periodontal conditions, emphasizing their differential diagnosis, prognosis and therapy.
