Tras trazar un breve panorama del concepto de cambio psíquico en la historia del psicoanálisis, el autor concibe el insight y la elaboración como dos aspectos inseparables de un mismo proceso que incluye el "desprendimiento" de los objetos internos patógenos y de las identificaciones patógenas. En este trabajo, ilustrado con dos viñetas clínicas de dos pacientes varones en análisis, se describe el cambio como una reestructuración identificatoria en el aparato psíquico que tiene lugar en el vínculo con el otro. Al mismo tiempo, el autor nos muestra cómo el proceso de cambio puede ser especialmente obstaculizado, entorpecido, dificultado en todo un conjunto de pacientes difíciles que carecen de los recursos yoicos necesarios para afrontar la ansiedad que les crea el vacío causado por la desidentificación y separación del objeto. En estos casos, el proceso analítico permite que el paciente pueda vivir experiencias de las que se vio carenciado en su desarrollo, y la función analítica tiene que realizar una función de complementariedad a través de la interpretación, que promueve el insight, y la asistencia al sí mismo, en donde el analista presta recursos yoicos que el paciente aún no tiene.

After providing a brief overview of the concept of psychic change in the history of psychoanalysis, the author envisions insight and working through as two inseparable aspects of the same process which includes "detachment" from the pathogenic internal objects and from pathogenic identifications. In this paper, illustrated by two clinical vignettes of male patients in analysis, change is described as an identificatory restructuring in the psychic apparatus which takes place in the link with the other. At the same time, the author shows us how the process of change may be especially resisted by a whole range of dif:ficult patients who are lacking the necessary ego resources to deal with the anxiety produced by the void caused by disidenti:fication and separation from the object. In these cases, the analytic process enables the patient to have experiences that were deemed to be lacking in his development, and the analytic function must perform a function of complementarity through interpretation, which fosters insight, and self-care, wherein the analyst provides ego resources which the patient does not yet possess.

Apres avoir tracé un bref aperçu du concept de changement psychique dans l'histoire de la psychanalyse, l'auteur conçoit l'insight et l'élaboration comme deux aspects inséparables d'un meme processus qui comprend le "décollement" des objets internes pathogenes et des identifications pathogenes. Dans ce travail, illustré avec deux vignettes cliniques de deux patients masculins en analyse, le changement est décrit comme une restructuration identificatoire dans l'appareil psychique qui a lieu dans le rapport a l'autre. En meme temps, l'auteur nous montre comment tout un ensemble de patients difficiles peut résister au processus de changement en raison du manque de ressources du moi nécessaires pour affronter l'anxiété que leur crée le vide causé par la désidentification et la séparation de l'objet. Dans ces cas, le processus analytique permet au patient de vivre des expériences qui lui ont manqué lors de son développement, et la fonction analytique doit réaliser une fonction de complémentarité au travers de l'interprétation, qui favorise l'insight et l'assistance au soi, ou l'analyste prete des ressources du moi que le patient n'a pas encore.
