Maduración cicatricial, control del linfedema y recuperación funcional de una extremidad inferior afectada por una gran cicatriz patológica

    Nuestro objetivo fue valorar la efectividad del uso combinado de distintas técnicas para madurar la extensa cicatriz patológica de una herida dehiscente por cierre por segunda intención, surgida tras la resección de liposarcoma gigante de extremidad inferior. Planificamos el control del dolor, la cura con apósitos especiales de silicona, asociados a compresión local; la reducción del linfedema, con inelasticidad en la descongestión y después medias de baja elasticidad; así como la rehabilitación fisioterapéutica de la extremidad.

    Utilizamos apósitos de silicona, vendas de tensión controlada, prendas de presoterapia con almohadillas de viscoelástica, y rehabilitación funcional de la extremidad. Como conclusión, hemos parcialmente reducido la profundidad de algunas zonas cicatriciales en valle, controlando el linfedema, y facilitando la recuperación de la elasticidad de la cicatriz retráctil de la ingle, que devolvió a la extremidad parte de la movilidad perdida. Podemos concluir que, la utilización conjunta de varias técnicas, al recuperar la elasticidad de la ingle, y liberar la tensión interna del linfedema, mantuvo a la extremidad afectada, y a la cicatriz, en buenas condiciones, lo que resultó crucial para una aceptable recuperación funcional de la extremidad afectada y la correcta maduración de la herida.

    Our objective was to evaluate the effectiveness of the joint use of different techniques to mature the extensive pathological scar from a wound dehiscent by close by second intention, arising after the resection of a big liposarcoma of lower extremity.

    We plan pain control, the cure with special silicone dressings, associated with local compression;

    lymphedema reduction, with inelasticity in decongestion and then half of low elasticity;

    as well as the rehabilitation physiotherapy of the extremity. We use silicone dressings, bandages of controlled voltage, garments of pressotherapy with pads of viscoelastic, and functional rehabilitation of the extremity. As a conclusion, we have partly reduced the depth of some areas scars in the valley, check the lymphedema, and facilitate the recovery of the elasticity of the retractable scar of the groin, which returned to the tip of the lost mobility. We can conclude that the joint use of several techniques, to retrieve the elasticity of the groin, and release the internal voltage of lymphedema, kept the affected limb, and scar, in good condition, which was crucial for an acceptable functional recovery of the affected extremity and the correct ripening of the wound.
