El diagnóstico etiológico de la insuficiencia cardíaca puede suponer un desafío para el clínico, quien se debe esforzar siempre en detectar el factor precipitante por las implicaciones pronósticas y terapéuticas que ello conlleva. A pesar de que la etiología es muy diversa y existen múltiples variables que influyen en la expresividad del síndrome clínico, una historia clínica detallada y el uso racional de los métodos diagnósticos disponibles hoy día nos permiten detectar, en la mayoría de las ocasiones, las alteraciones estructurales, congénitas o adquiridas que suponen el sustrato necesario para su desarrollo y los factores directamente responsables de las descompensaciones.

The etiological diagnosis of heart failure can be a challenge for the clinician, who should always strive to detect the precipitating factor due to the prognostic and therapeutic implications that it entails. Although the etiology is very diverse and there are multiple variables that influence the expression of the clinical syndrome, a detailed clinical history and the rational use of the diagnostic methods available today allow in most occasions to detect the structural anomalies, congenital or acquired, that represent the mandatory alteration for its development and the factors directly responsible for the decompensation.
