    En las últimas décadas se ha producido un notable avance en el manejo del cáncer colorrectal metastásico (CCRm). La combinación de múltiples fármacos citotóxicos y las nuevas terapias dirigidas ha permitido alcanzar una supervivencia global superior a los 30 meses, así como aumentar la tasa de resección quirúrgica con intención curativa de la enfermedad metastásica. Actualmente, la elección de la primera línea de tratamiento sistémico se basa principalmente en el objetivo terapéutico, el estado general del paciente, y el estado mutacional de los genes RAS y BRAF. La integración de la quimioterapia con la cirugía y otras técnicas ablativas debe realizarse de forma individualizada dentro de un comité multidisciplinar de tumores.

    In the last decades, there has been a notable advance in the management of metastatic colorectal cancer. The combination of multiple cytotoxic drugs and the new targeted therapies has achieved an overall survival of more than 30 months, as well as an increase in the rate of curative-intent surgical resections of metastatic disease. Currently, the choice of the first line of systemic treatment is based on the therapeutic objective, the general condition of the patient, and the mutational status of the RAS and BRAF genes. The integration of chemotherapy with surgery and other ablative techniques must be performed individually within a multidisciplinary tumor committee.
