El hígado es la localización metastásica más frecuente en los pacientes diagnosticados de cáncer de colon. Para aquellos que presentan metástasis hepáticas técnicamente resecables, la intención del tratamiento es curativa con el objetivo de conseguir una resección completa (R0), con supervivencia a 10 años que alcanza el 20-30%. La mayoría de los pacientes metastásicos tienen enfermedad irresecable. Sin embargo, para aquellos con afectación limitada al hígado, el tratamiento quimioterápico está incrementando las tasas de resecabilidad en pacientes seleccionados. No está claro cuál es la mejor combinación terapéutica dado que pocos ensayos clínicos lo han evaluado.
The liver is the most common metastatic location in patients diagnosed with colon cancer. The primary goal for patients who present with technically resectable liver metastases is clearly cure, with the primary goal R0 resection. The majority of patients diagnosed with metastatic colorectal disease have unresectable disease. However, for those with liver-limited unresectable disease chemotherapy is being increasingly considered in highly selected cases in an attempt to downsize colorectal metastases and convert them to a resectable status. There is uncertainty surrounding the best combination to use as only few trials have addressed this specifically.
