Asociaciones Relacionadas con las Causas de Muerte en la Artritis Reumatoide: Estudio Descriptivo

    OBJETIVO: Determinar la tasa de mortalidad estandarizada (Tl\IE) y las causas de muerte atribuibles a la enfermedad o su tratamiento en pacientes con artritis reumatoide. MATERIAL Y MÉTODOS: Se realizó un estudio observacional, retrospectivo, longitudinal y descriptivo donde se evaluó historias clínicas correspondientes a pacientes con AR (según criterios de la ARA/CAR), que asistieron a la Sección de Reumatología del Hospital Central de la Fuerza Aérea del Perú, desde julio de 1976 hasta abril de 1993. Para el cálculo de la TME se utilizó como tasa de mortalidad poblacional durante el período de estudio 9,5/1 000 habitantes, según datos del Ministerio de Salud. RESULTADOS: Se atendió 245 pacientes en 17 años, de los cuales fallecieron 38 (15%). Éstos fueron los que menos cumplieron con el tratamiento indicado (24 % contra 14 %). Las causas de muerte según la clasificación propuesta por Wolfe fueron: atribuibles a la enfermedad o a su tratamiento 44% (16 pacientes), no relacionadas 36% (13 pacientes), contributorias 16 % (seis pacientes). No se encontró información en tres pacientes. CONCLUSIONES: La TME para el grupo de estudio es 1,63. Las causas más frecuentes de muerte en la AR son las atribuibles (compromiso pulmonar, las debidas al tratamiento), seguidas de las contributorias (infecciosas) y las no relacionadas (neoplasias primarias o metastásicas).

    OBJETIVE:To determine the standardized mortality ratio (Tl \ IE ) and causes of death attributable to the disease or its treatment in patients with rheumatoid arthritis. MATERIAL AND METHODS : An observational , retrospective, longitudinal and descriptive study of pediatric patients with RA was assessed medical records ( criteria of the ARA / CAR ) , who attended the Rheumatology Section, Central Hospital of Air Force was conducted Peru , from July 1976 to April 1993. For the calculation of TME was used as population mortality rate during the study period 9.5 /1 000, according to the Ministry of Health. RESULTS : 245 patients were attended in 17 years, of whom 38 died (15%). They were least satisfied with the prescribed treatment (24 % vs. 14%). The causes of death according to the classification given by Wolfe were attributable to the disease or its treatment 44 % (16 patients) , unrelated 36 % (13 patients) , Contributory 16% (six patients). No information was found in three patients. CONCLUSIONS: TME for the study group is 1.63 . The most frequent causes of death in RA are attributable (lung involvement due to treatment), followed by the Contributory (infectious) and unrelated (primary or metastatic tumors).
