Resumen de Herida compleja y cierre por segunda intención. ¿la terapia de presión negativa es buena opción?

  Presentamos el caso clínico de una mujer de 63 años, sin patologías de interés, con una herida inciso contusa en extremidad inferior, complicada, tras un abordaje inicial discutible, y el posterior manejo terapéutico hasta lograr su resolución.

  La descarga bacteriana, junto al control y eliminación del biofilm, producen que la herida compleja evolucione favorablemente. En este caso, la asociación de mallas de hidrocoloides con apósitos de fibras y plata no resultó eficaz para eliminar la biofilm. La Terapia de Presión Negativa (TPN), la descarga bacteriana realizada con los fomentos, y posterior desbridación mecánica del lecho, junto al cadexómero Iodado, primero, y posteriormente los apósitos de cloruro dialquilcarbamilo, resultaron efectivos para resolver la complejidad de la herida.

  We present the clinical case of a 63-year-old woman, without pathologies of interest, with a contusive wound in the lower limb, complicated, after an initial debatable approach, and the subsequent therapeutic management until its resolution is achieved. The bacterial discharge, together with the control and elimination of the biofilm, produce that the complex wound evolve favorably. In this case, the association of hydrocolloid meshes with fiber and silver dressings was not effective in eliminating biofilm. Negative Pressure Therapy (NPT), the bacterial discharge carried out with the fomentation, and subsequent mechanical debridement of the bed, together with the cadexomer first, and then the dressings of chloride dialquilcarbamilo, were effective to resolve the complexity of the wound.
