    • The unprecedented changes happening in the Americanhealthcare system have many on high alert as they try toanticipate legislative actions. Significant efforts to movefrom volume to value, along with changing incentives andalternative payment models, will affect practice and thehealth system budget. In tandem, growth in the populationaged 65 and older is celebratory and daunting. The JohnA. Hartford Foundation is partnering with the Institute forHealthcare Improvement to envision an age-friendly healthsystem of the future. Our current prototyping for newways of addressing the complex and interrelated needs ofolder adults provides great promise for a more-effective,patient-directed, safer healthcare system. Proactive modelsthat address potential health needs, prevent avoidableharms, and improve care of people with complex needsare essential. The robust engagement of family caregivers,along with an appreciation for the value of excellent com-munication across care settings, is at the heart of ourwork. Five early-adopter health systems are testing theprototypes with continuous improvement efforts that willstreamline and enhance our approach to geriatric care.
