Le disease management correspond à un système coordonné de communication et d’intervention auprès des patients dont la maladie nécessite une réelle autogestion. Une telle stratégie d’accompagnement a pour but d’améliorer les connaissances et l’autonomie du patient avec, pour conséquence, une meilleure qualité de vie et une diminution des coûts liés à sa maladie. Dans cette perspective, l’Assurance maladie propose aux patients diabétiques le service Sophia, disponible via internet. Ce dispositif d’accompagnement est amené à concerner, à l’avenir, d’autres pathologies chroniques.

Disease management corresponds to a coordinated system of communication and support for patients whose illness needs to be self-managed. Such a support strategy aims to improve patients’ knowledge and autonomy offering them a better quality of life and reducing the costs related to their disease. To this end, the French health insurance system has created the Sophia service aimed at patients with diabetes and available via the internet. This support programme will in future be extended to other chronic diseases.
