    • Frailty for older adults connotes vulnerability. The Clinical Frailty Scale, a frailty index, a frailty phenotype, and the Identification of Seniors at Risk are commonly used to identify frailty in the emergency department. An older patient identified as frail should receive an interdisciplinary assessment, which includes a review of their medications and a review for common geriatric problems. A care plan should be developed for frail older patients, and should take into account their individual goals and preferences.
