Resumen de Optimización de la farmacoterapia en paciente diabético tipo 2 por cambio de insulina

  Varón de 74 años con Diabetes Mellitus tipo 2 (DM2) e hipertensión arterial (HTA), paciente habitual de la farmacia en el servicio de seguimiento farmacoterapéutico que en Octubre de 2017 acude a la farmacia para comentarnos que está teniendo situaciones de hipoglucemias frecuentes.

  Las cifras de hemoglobina glicosilada en ese momento del paciente estaban en 7.9 %.

  El tratamiento del paciente era Novomix 30 (22 UI/0/22 UI), 6 UI Novorapid antes de las comidas si glucemia >250 mg/dl, Ristfor (1-0-1), atorvastatina 20 mg (0-0-1), candesartán 8 mg (1-0-0) y ácido acetilsalicílico 100 mg (1-0-0).

  El paciente nos expresa su preocupación ya que sufre situaciones de hipoglucemia con picos de hiperglucemia.

  Además, el paciente nos demanda la realización de educación diabetológica, desarrollando de forma conjunta (paciente-farmacéutico) un trabajo de empoderamiento, que tiene como objetivo fundamental el autocontrol de la enfermedad, labor fundamental en el éxito del buen pronóstico de la misma.

  In the context of the healthcare process known in Spain as pharmacotherapeutical follow-up, the function of the clinical pharmacist is to optimize the patient’s pharmacotherapy, so that with the correct usage of the medicines that the patient needs, the therapeutic objectives wanted are reached reducing the side effects. This assistance practice normally begins when the patient asks for it when he or she has got a reason for consultation that has to be solved. We can not be content with solving only this problem but we must offer an integrated approach to his or her needs. So as to do it, we have to evaluate his or her pharmacotherapy and analyze his or her beliefs, fears and expectancy qualitatively in the relation between the patient and his or her medication and illness.
