Resumen de Dependencia al alcohol, patología dual y funcionamiento neurocognitivo: Una revisión necesaria

  Introducción: El efecto dañino causado por el consumo crónico de alcohol sobre el cerebro y los procesos cognitivos ha sido sistemáticamente descrito en la literatura durante décadas. Sobre las particularidades del funcionamiento cognitivo en pacientes con patología dual existen actualmente pocos resultados. Objetivo: Realizar una valoración crítica de las evidencias disponibles sobre la relación entre Patología Dual (PD) y funcionamiento neuropsicológico. Material y Método: Se realizó una revisión sistemática sobre bases de datos digitales. El tratamiento de datos fue mediante el análisis de contenido de tipo directo. Desarrollo: El estudio de las alteraciones cognitivas y neuropsicológicas en PD no puede realizarse al margen del tipo de trastorno psicopatológico que presenta el paciente con dependencia al alcohol. En ese campo apenas se han publicado artículos que describan los daños neuropsicológicos en pacientes con PD. Se ha comprobado que las mayores dificultades se aprecian específicamente en las funciones ejecutivas, particularmente en los dominios de shifting, updating e inhibición. Conclusiones: La evidencia sobre las alteraciones neurocognitivas en pacientes con PT es reducida. Existen interesantes estudios que sugieren la posibilidad de que la comorbilidad psiquiátrica en la dependencia alcohólica tenga un efecto mucho más grave sobre el funcionamiento neuropsicológico, en comparación con pacientes que solo presentan una de las dos alteraciones aisladas. El diagnóstico psicopatológico que coexiste con la dependencia, es un factor que impacta de modo diferente en la magnitud y el tipo de alteración neurocognitivaque presentan los pacientes duales. Palabras claves: patología dual, neuropsicología, dependencia alcohólica, procesos cognitivos.

  Introduction: The harmful effect on the brain and the cognitive processes, caused by the chronic consumption of alcohol has been systematically described in literature for decades. At present, there are few results about the distinctive features of cognitive functioning in patients with dual pathology.  Objective: To carry out a critical assessment of the available evidence on the relationship between Dual Pathology (DP) and neuropsychological functioning. Material and Method: A systematic review of the digital databases was carried out.  The data processing was made through a direct content analysis.  Development: The study of cognitive and neuropsychological alterations in DP cannot be made regardless of the type of psychopathological disorder presented by the patient with alcohol dependence.  In this field, just a few articles that describe the neuropsychological damage in patients with DP have been published.  It has been confirmed that the greatest difficulties are specifically seen in executive functioning, particularly in shifting, updating, and inhibition. Conclusions: The evidence of neurocognitive alterations in patients with DP is reduced. There are interesting studies that suggest the possibility that psychiatric comorbidity in alcohol dependence has a much more serious effect on neuropsychological functioning, compared with patients who only present one of the two isolated alterations. The psychopathological diagnosis that coexists with dependence is a factor that impacts differently on themagnitude and the type of neurocognitive alterations that dual patients present. Keywords: dual pathology, neuropsychology, alcohol dependence, cognitive process.
