Resumen de Nursing care in adult patients with extracorporeal membrane oxygenation (ECMO): Systematic review

  Objetivos: El objetivo principal de este artículo fue revisar la literatura acerca de los cuidados específicos que el profesional de enfermería realiza a un paciente adulto sometido a ECMO, identificando la existencia de planes de cuidados estandarizados que optimicen la calidad asistencial y favorezcan la continuidad de los cuidados.

  Métodos: Se llevó a cabo una búsqueda en las siguientes bases de datos: CINAHL, CUIDEN PLUS, EMBASE, LILACS, MEDLINE, PUBMED. Todas las listas de referencias fueron importadas a un gestor bibliográfico (RefWorks), herramienta utilizada para eliminar documentos duplicados. Los criterios de inclusión fueron: (1) artículo científico original, (2) texto completo publicado en español o inglés. Los criterios de exclusión fueron: (1) artículos publicados en otros idiomas, (2) artículos en los que las intervenciones realizadas sobre los pacientes en ningún momento implican la participación de enfermería.

  Resultados: Se realizó revisión de 11 artículos, analizándose el rol del personal de enfermería y los cuidados al paciente con ECMO. Cabe destacarla importancia de la formación previa en la técnica y la evaluación exhaustiva de todos los sistemas fisiológicos y del circuito ECMO, sin olvidar el apoyo familiar.

  Conclusiones: La enfermera debe proporcionar cuidado directo al paciente con ECMO y al circuito, revisar atentamente todos los sistemas del organismo y atender el cuidado del familiar. Por tanto, adquiere un papel primordial en la coordinación de la atención y en la vigilancia del paciente en ECMO.

  Objectives: The main objective of this article was to review the literature about the specific nursing care to adult patients with ECMO, identifying the existence of standardized care plans that optimize the quality of the care and favor the continuity of the care.

  Methods: A search was carried out in the following databases: CINAHL, CUIDEN PLUS, EMBASE, LILACS, MEDLINE, PUBMED. All reference lists were imported to a bibliographic manager (RefWorks), a tool used to eliminate duplicate documents. The inclusion criteria were: (1) original scientific article, (2) full text published in Spanish or English. The exclusion criteria were: (1) articles published in other languages, (2) articles in which the interventions performed on patients did not involve nursing participation.

  Findings: A review of 11 articles was carried out, analyzing the role of nurses and the care of patients with ECMO. It is important to highlight the importance of prior technical training and the thorough evaluation of all physiological systems and the ECMO circuit, without forgetting family support.

  Conclusions: The nurse must provide direct care to the patient with ECMO and to the circuit, carefully review all physiological systems and pay attention to the family members’ care. Therefore, the nurse acquires a primary role in the coordination of care and in the monitoring of the patient with ECMO.
