  • In May 2014, 13 people were injured in a fire at a museum that resulted from a chemistry demonstration called the Flaming Tornado. The U.S. Chemical Safety and Hazard Investigation Board studied this incident and reported several issues—notable was that no hazard analysis had been performed. The case study of this incident provides a vehicle with which to teach the principles of hazard analysis and risk minimization.
