    • Aims: Treatment with olanzapine was evaluated in 27 patients with repeated cocaine consumption without psychotic symptomatology on a methadone program. Patients and methods: Patients were selected from those who presented recurrent positive cocaine urine analysis over a 6-month period. Patients who consented to inclusion in the study received olanzapine 10 mg/d of for the following 6 months. Results: A very significant decrease in positive cocaine and heroin urine analysis was observed (cocaine 36.73% versus 2.38%; heroin 16.60% versus 4.35%). Likewise, a decrease was found in the mean dose of methadone between the previous 6 months and the 6 months on olanzapin. Five of the 27 patients withdrew from the study. Discussion: Olanzapine administration may favor compliance with the methadone program allowing a reduction in the daily dose of methadone and an important decrease in the consumption of cocaine and/or heroin.
