Resumen de Fractura por Insuficiencia de los Platillos Tibiales en Artritis Reumatoide

  En el presente artículo reportamos el desarrollo de una fractura por insuficiencia de los platillos tibiales en una paciente con artritis reumatoide de larga evolución.  En la artritis reumatoide existen una serie de factores que alteran la resistencia elástica del hueso como: osteoporosis periarticular o generalizada, terapia esteroidea, deformación angular de las extremidades y contracturas en flexión; los cuales pueden condicionar el desarrollo de fracturas por insuficiencia. Adicionalmente el dolor y la incapacidad funcional que provoca la fractura puede ser atribuido a la artritis reumatoide. Se remarca la importancia de incluir a la fractura por insuficiencia de los platillos tibiales en el diagnóstico diferencial de pacientes con dolor localizado en rodillas y patología ósea o reumática que se asocie con osteoporosis u otra causa de pérdida de la resistencia elástica del hueso.

  The ocurrence of insufficiency fractures in patients with long-standing, rheumatoid arthritis has not been sufficiently emphasized.  Osteoporosis due to rheumatoid arthritis, corticosteroid therapy, contracture and angular deformity of the extremity, combine to predispose to the ocurrence of the insufficiency fractures in these patients. Additionally, the pain and disability caused by the fracture is often attributed to rheumatoid joint involvement, masking the diagnosis of insufficiency fracture. The fracture may not be visible on radiographs near the onset of symptoms, and the bone scanning can help in make an early diagnosis.
