Evaluación y pertinencia de la consulta dermatológica ambulatoria espontánea durante la cuarentena total por COVID-19 en Chile

    Antecedentes: Se ha informado que la atención dermatológica puede funcionar como un vector para la transmisión del SARS-CoV-2 y que la consulta ambulatoria debería posponerse durante el periodo pandémico; sin embargo, en algunos casos, según la gravedad, por intervención quirúrgica o las limitaciones de la teledermatología, la consulta ambulatoria es pertinente. A la fecha se han realizado pocos estudios sobre las consultas durante la pandemia. Objetivo: Caracterizar la población que acude a consulta presencial en un servicio dermatológico ambulatorio y evaluar su pertinencia durante el periodo de confinamiento por la pandemia de COVID-19. Método: Estudio clínico observacional transversal que incluyó los pacientes que acudieron a consulta dermatológica ambulatoria espontánea entre mayo y junio de 2020. Se registraron la edad, el sexo, el diagnóstico y la pertinencia de la consulta. Resultados: Se incluyeron 236 sujetos, de los que 111 eran de sexo masculino. Un 5.5% (n = 13) tuvieron una edad ≥ 65 años, el 81.3% entre 15 y 64 años, y el 13.1% < 15 años. La consulta más frecuente fue el acné (n = 33, 19%), seguido de la dermatitis de contacto irritativa (n = 27, 11.44%) y las verrugas (n = 22, 9.3%). Se registró que en 95 (40.25%) pacientes la consulta fue pertinente. Conclusiones: En un 40.25% de los pacientes que consultaron en el primer periodo de confinamiento total fue pertinente la consulta ambulatoria presencial, mientras que un 59.75% no la requerían. Es esencial educar a la población sobre cuándo consultar e instruir sobre otras opciones, como la teledermatología, con el fin de disminuir el riesgo de transmisión.

    Background: It has been reported that dermatological care can function as a vector for the transmission of SARS-CoV-2 and outpatient consultation should be postponed during the pandemic period, however, in some cases, depending on severity, surgical intervention and limitations in teledermatology, spontaneous outpatient consultation is pertinent. To date, few studies have been conducted on consultations during the pandemic. Objective: Characterize the population that attends a face-to-face consultation in an outpatient dermatological service and evaluate its pertinent, during the confinement period, in the context of a COVID-19 pandemic. Method: Cross-sectional observational study, including patients who attended a face-to-face outpatient consultation between May and June, 2020. Age, sex, diagnosis and pertinence of the query. Results: It included 236 subjects, 111 were male. An 5.5% (n = 13) of the patients were older than or equal to 65 years, 81.3% between 15 and 64 years and 13.1% less than 15 years. The most frequent consultation was acne (n = 33, 19%), followed by irritant contact dermatitis (n = 27, 11.44%) and warts (n = 22, 9.3%). It was registered that 95 (40.25%) patients their consultation was pertinent. Conclusions: 40.25% of the patients who consulted in the first period of total quarantine their outpatient consultation was pertinent in person versus 59.75% who did not require it. It is essential that the population is educated on when to consult and instruct on other options such as teledermatology, in order to reduce the risk of transmissibility.
