    Caso clínico de un paciente que acude a urgencias por dolor en la extremidad inferior izquierda con frialdad y parestesia. En la exploración general se observa que el paciente está consciente, orientado, normocloreado y normohidratado, presenta frialdad y ausencia de pulso pedio y tibial posterior en extremidad inferior izquierda con diferencia de temperatura respecto de la otra extremidad inferior. Se solicita colaboración a cirugía vascular, tras ser valorado por estos proponen ingreso para estudio en hospitalización. Con los resultados del estudio y siendo el tratamiento ineficaz proponen doble intervención quirúrgica, By Pass femoro-poplíteo de la extremidad inferior derecha y simpatectomía lumbar izquierda. Se realiza la valoración enfermera mediante las 14 Necesidades de Virginia Henderson y un plan de cuidados enfermeros durante el ingreso que se va modificando dependiendo de sus necesidades en cada momento, antes de la intervención y después.

    Finalmente el paciente se va de alta a su domicilio con un alto nivel de independencia para las actividades instruméntales de la vida diaria y sin dolor por el que acudió a urgencias.

    Clinical case of a patient going to the emergency because he has pain in his left lower extremity with coldness and paresthesia. In the general medical examination it is observed that the patient is conscious, oriented, normocloreated and normohydrated, he presents coldness and absence of pedio and posterior tibial pulse in the left lower extremity with temperature difference with respect to the other lower limb. Collaboration is requested to vascular surgery, after being assessed by vascular doctors propose admission for study in hospitalization. With the results of the study and being the ineffective treatment proposed by the vascular surgeons double surgical intervention, femoro-popliteal bypass of the right lower extremity and left lumbar sympathectomy. The nurse evaluation is done through the 14 Needs of Virginia Henderson and a plan of nursing care during the admission that is modified depending on his needs at each moment, before the intervention and after. Finally, the patient goes home to his home with a high level of independence for the daily activities and without pain for which he went to the emergency room.
