    Cada vez son más las personas que pasan los últimos años de su vida institucionalizados y en el área de su farmacoterapia hallamos efectos adversos. Para mejorar en su salud es necesario favorecer lazos de comunicación en el espacio sociosanitario entre distintos agentes sociales y sanitarios. Se deben impulsar y generar lazos entre administraciones en torno al paciente y sus medicamentos para garantizar un sistema sostenible y un confort a la ciudadanía.

    More and more people have spent the last years of their lives institutionalized and in the area of their pharmacotherapy we find adverse effects. To improve their health it is necessary to foster communication ties in the social-health space between different social and health actors. Relationships between administrations around the patient and their medicines should be promoted and generated to ensure a sustainable system and comfort to citizens.

    Gero eta jende gehiagok ematen ditu bere bizitzako azken urteak instituzionalizatuta eta farmakologia arloan albo-ondorioak aurkitzen ditugu. Beren osasuna hobetzeko, beharrezkoa da gizarte-osasuneko espazioan gizarte-osasuneko eta gizarte-osasuneko aktore ezberdinen arteko harremanak bultzatzea. Administrazioen arteko harremanak sustatu behar dira pazienteen eta haien medikamentuen inguruan.
