Control del Hipotiroidismo durante la gestación por la matrona, en atención primaria

    Introducción: El hipotiroidismo durante la gestación pone en compromiso el bienestar entre binomio materno fetal, está descrito un aumento de la morbimortalidad a lo largo de la gestación y a largo plazo en el recién nacido. Por ello, reviste gran importancia desde la primera consulta de la matrona, conocer la función tiroidea materna.Objetivo: Identificar de forma precoz el hipotiroidismo en la consulta de la matrona de atención primaria.Metodología: Se llevó a cabo una revisión bibliográfica en diferentes bases de datos, de acuerdo a 2 áreas temáticas (hipotiroidismo y gestación), combinando los descriptores DECs y operadores booleanos. La búsqueda se acotó a estudios de los últimos 5 años, con acceso al texto completo e idioma español o inglés.Resultados: Se seleccionaron 17 artículos, de los cuales 8 tratan sobre las alteraciones de la función tiroidea y sus efectos en la gestación, 7 se centran en el estudio del hipotiroidismo, sus tipos y consecuencias en la gestación y 11 de ellos incluyen, además, las repercusiones de la disfunción tiroidea gestacional en la descendencia.Conclusiones: Solicitar un cribado precoz en la primera consulta de la matrona, para conocer la disfunción hipotiroidea y tratarla cuanto antes, podría disminuir la morbimortalidad asociada entre el binomio maternal fetal.

    Introduction: Hypothyroidism during pregnancy endangers the maternal-fetal wellness, increased morbidity and mortality has been described throughout gestation and in the long term in the newborn. Therefore, it has great importance from the first midwife consultation, know the maternal thyroid function.Objective: To identify early hypothyroidism in the midwife consultation in primary care.Methodology: A bibliographic review was carried out in different databases, according to 2 thematic areas (hypothyroidism and pregnancy), combining the DECs descriptors and Boolean operators. The search was limited to studies from the last 5 years, with access to full text and Spanish or English language.Results: 17 articles were selected, 8 of which are about thyroid function alterations and their effects on pregnancy, 7 focus on the study of hypothyroidism, types and consequences on pregnancy, and 11 of them also include the repercussions of gestational thyroid dysfunction in offspring.Conclusions: Request an early screening, in the midwife’s first consultation, to know the hypothyroid dysfunction and treat it as soon as possible, could decrease the associated morbidity and mortality in the maternal fetal set.
