Resumen de La combinación: inmunoterapia e inhibidores de tirosina quinasa como primera línea del cáncer renal metastásico

En la última década se han conseguido importantes avances en el tratamiento del cáncer renal. Durante muchos años la nefrectomía fue el tratamiento estándar de elección, pero el creciente conocimiento de las vías moleculares de este tumor ha llevado al desarrollo de múltiples agentes terapéuticos que han incrementado la supervivencia global (SG) de estos pacientes de 7 a 30 meses con la adecuada secuencia terapéutica. El cáncer renal presenta una gran expresión de factor de crecimiento endotelial vascular (VEGF) que justifica los importantes beneficios del empleo de fármacos antiangiogénicos en primera línea. Debido a la actividad demostrada de la inmunoterapia en segunda línea (anti- PD-1, nivolumab) se ha querido investigar la actividad de otros inhibidores del punto de control inmune (anti- PD-1 pembrolizumab y anti- PD-L1 avelumab) en combinación con el inhibidor de tirosina quinasa (ITK) axitinib en dos ensayos clínicos fase 3 (KEYNOTE-426 and JAVELIN Renal 10). Resumimos los resultados de ambos ensayos y analizamos los recientes cambios en el tratamiento de primera línea de cáncer renal.

In the last decade many important advances have been done in renal cancer treatment. During many years nephrectomy was the standard of care but the growing knowledge of molecular pathways has allowed the development of multiple therapeutic agents that have increased the overall survival (OS) of these patients from 7 to 30 months with the appropriate therapeutic sequence. Renal cancer has a high expression of vascular endothelial growth factor (VEGF) that justifies the important benefits of the use of anti-angiogenic drugs in front line. Due to the demonstrated activity of second-line immunotherapy (anti-PD-1, nivolumab), the activity of other inhibitors of the immune control point (anti-PD-1 pembrolizumab and anti-PD-L1 avelumab) has been investigated in combination with the tyrosine kinase inhibitor (ITK) axitinib in two phase 3 clinical trials (KEYNOTE-426 and JAVELIN Renal 10). We summarize the results of both trials and analyze the recent changes in the first line treatment of renal cancer.
