Terapia ocupacional en la fase preprotésica del paciente amputado de miembro superior.

    Tras la amputación de una extremidad y la indicación médica de inicio del tratamiento en terapia ocupacional, el objetivo principal a largo plazo será mejorar la funcionalidad y recuperar la independencia en las AVD. Esta fase implica procesos muy importantes para la posterior utilización y aceptación de la prótesis. El terapeuta ocupacional debe tener en cuenta el dolor y conocer los métodos para mejorar la situación del paciente. Una vez iniciado el tratamiento se dará forma, se desensibilizar el muñón, aprenderá el cuidado del mismo y la forma de vendarlo, mantendrá tanto la movilidad del lado afectado como fortalecer el sano, aprenderá técnicas de manejo del dolor y se entrenará en actividades unimanuales, bimanuales y cambio de dominancia si fuera necesario. OBJETIVO: Conocer los métodos de tratamiento realizados desde Terapia Ocupacional en la fase preprotésica en pacientes que han sufrido amputaciones de extremidad superior. METODOLOGÍA: Se ha realizado una búsqueda bibliográfica en las bases de datos PubMed, Dialnet y Uptodate, así como en Google Académico, utilizando como términos de búsqueda: terapia ocupacional, amputación, fase preprotésica, autocuidado muñón, vendajes. Exclusión: artículos con acceso restringido. RESULTADOS: Conocer con mayor detalle las fases previas a la protetización de una extremidad y posteriores al trauma o postoperatorias, dentro del tratamiento de terapia ocupacional con pacientes amputados de extremidad superior. CONCLUSIONES El tratamiento de un amputado implica un abordaje interdisciplinar coordinado, donde resulta imprescindible la figura del terapeuta ocupacional por su aportación a la preparación del muñón para la siguiente fase. La fase preprotésica es una fase clave puesto que sucede tras el momento traumático o postoperatorio, y si se realiza correctamente, podremos avanzar a la siguiente fase llamada fase protésica.

    After the amputation of a limb and the medical indication to start treatment in occupational therapy, the main long-term objective will be to improve functionality and regain independence in ADL. This phase involves very important processes for the subsequent use and acceptance of the prosthesis. The occupational therapist must be aware of pain and know the methods to improve the patient’s situation. Once the treatment has started, it will be shaped, the stump will be desensitized, you will learn how to care for it and how to bandage it, you will maintain both the mobility of the affected side and strengthen the healthy one, you will learn pain management techniques and you will be trained in one-manual activities, bimanual and change of dominance if necessary. OBJECTIVE: To know the treatment methods carried out from Occupational Therapy in the pre-prosthetic phase in patients who have suffered upper limb amputations. METHODOLOGY: A bibliographic search was carried out in the PubMed, Dialnet and Uptodate databases, as well as in Google Scholar, using as search terms: occupational therapy, amputation, pre-prosthetic phase, stump self-care, bandages. Exclusion: articles with restricted access. RESULTS: Know in greater detail the phases prior to the prosthetics of a limb and after the trauma or postoperative, within the occupational therapy treatment with upper extremity amputee patients. CONCLUSIONS The treatment of an amputee implies a coordinated interdisciplinary approach, where the figure of the occupational therapist is essential for his contribution to the preparation of the stump for the next phase. The pre-prosthetic phase is a key phase since it happens after the traumatic or postoperative moment, and if it is done correctly, we can advance to the next phase called the prosthetic phase.
