Descentralización sanitaria, triunfo del nacionalismo y gestión de la COVID-19 en España

    Este trabajo pretende abordar la conmoción provocada por la pandemia acaecida por el SARS-Cov-2, dada la afectación de la salud pública y, también, del Sistema Nacional de Salud. Es evidente que la COVID 19 marcará un antes y un después en la sanidad española, donde será necesaria una reorganización de esta en torno al Sistema Nacional de Salud. A través de la dotación de recursos, reformas en la gobernanza, la gestión y organización. La crisis de salud pública ha desbordado la capacidad del sistema sanitario debido, en gran parte, a sus carencias organizativas, así como de sus sistemas de información y, sobre todo, por una gobernanza carente de coordinación y cooperación sanitaria interautonómica. La inexistencia de un Sistema Nacional de Salud que reforzara el Ministerio de Sanidad y le otorgase solidez y capacidad de coordinación y cooperación, sí; pero también, la falta de una capacidad normativa que permita crear un sistema de información independiente, ajeno a los intereses autonómicos; han sido las principales causas. Con su creación, pretende aprenderse del error. A ello, puede unirse también la creación de reservas nacionales de carácter estratégico de material crítico, una red sólida de hospitales nacionales, con una clara estrategia en materia de investigación sanitaria. Una reorganización estatal permitiría una presencia mucho más activa y fuerte en el escenario mundial, especialmente en el marco de la OMS y en el Centro Europeo para la Prevención y el Control de las Enfermedades. Primará así la cooperación internacional y la diplomacia sanitaria que, a partir de esta tragedia, deberá plantearse como una parte de importancia estratégica capital para las relaciones exteriores. Este motivo ha llevado a dos constitucionalistas aun cuando se desconocen con exactitud las consecuencias catastróficas de la pandemia a plantear debates y propuestas sobre la descentralización de la sanidad y cuáles son los retos que habremos de abordar en un futuro muy próximo.

    This work aims to address the shock caused by the pandemic caused by SARS-Cov-2, given the impact on public health and, also, on the National Health System. It is evident that COVID 19 will mark a before and after in Spanish healthcare, where it will be necessary to reorganize it around the National Health System. Through the provision of resources, reforms in governance, management and organization. The public health crisis has overwhelmed the capacity of the health system due, in large part, to its organizational shortcomings, as well as its information systems and, above all, due to a governance that lacks coordination and inter-autonomous health cooperation. The non-existence of a National Health System that reinforces the Ministry of Health and gives it solidity and capacity for coordination and cooperation, yes; but also, a regulatory capacity that allows the creation of an independent information system, alien to regional interests; has been the cause of causes. With its creation, it intends to learn from the error, to which can also be added the creation of strategic national reserves of critical material, a solid network of national hospitals, with a clear strategy in health research. A state reorganization would allow a much more active and strong presence on the world stage, especially within the framework of the WHO and the European Center for Disease Prevention and Control. Thus, international cooperation and health diplomacy will prevail, which, after this tragedy, should be considered as a part of capital strategic importance for foreign relations. This reason has led two constitutionalists, even when the catastrophic consequences of the pandemic are unknown, to raise debates and proposals on the decentralization of health and what are the challenges that we will have to address in the very near future.
