Resumen de Protocolo de adquisición de spect-ct pulmonar en pacientes Covid+. Cuidados de enfermeria

  Objetivo Adaptar el protocolo estándar del SPECT-CT pulmonar a la situación de pandemia provocada por el virus SARS-CoV-2.

  Describir los cuidados de enfermería necesarios para realizar el procedimiento.

  Material y método Adaptamos franjas horarias para evitar el cruce de pacientes “No Covid” con pacientes “Covid +”. Despejamos la sala de exploración y aislamos debidamente la mesa de exploración con cada paciente. Usamos equipos EPI adecuados y comprobamos que los pacientes “Covid+” vienen bien protegidos para evitar contaminación. Asimismo, adaptamos los protocolos de enfermería para un correcto manejo de dichos pacientes.

  Eliminamos la fase de ventilación del protocolo estándar para evitar la diseminación del virus. La exploración empieza administrando 99mTc-MAA en dosis estándares para adquirir la fase de perfusión. Seguidamente realizamos la CT.

  Hemos seguido el Plan de Cuidados pautado comprobando en todo momento el confort y el estado general del paciente.

  Resultado Entre marzo y junio de 2020 realizamos 405 SPECT-CT pulmonar (203 mujeres, 202 hombres; rango de 15-96 años) con alta sospecha de TEP. 311 pacientes (77%) estaban ingresados y 94 (23%) fueron ambulatorios.

  Del total acudieron sin PCR 72 pacientes (18%) mientras que al resto (333 – 82%) si se les realizó, siendo positiva en 67 casos (20%).

  En 137 casos (34%) el SPECT-CT fue positivo para TEP; de éstos, 56 (41%) fueron diagnosticados de Covid+ con la CT previa.

  Conclusiones El Hospital Germans Trias i Pujol es centro de referencia en la detección de T.E.P. en Cataluña.

  El manejo del paciente con “Covid+” ha dificultado la labor de Enfermería en la práctica diaria. Aplicando el protocolo adecuado y ofreciendo cuidados enfermeros precisos hemos podido realizar los estudios de forma óptima con un buen rendimiento diagnóstico.

  Target Adapt the standard lung SPECT-CT protocol to the pandemic situation caused by the SARS-CoV-2 virus.

  Describe the nursing care necessary to perform the procedure.

  Material And Method We adapt time slots to avoid crossing “No Covid” patients with “Covid +” patients. We clear the examination room and properly isolate the examination table with each patient. We use appropriate PPE equipment and check that the “Covid+” patients came well protected to avoid contamination. Likewise, we adapt the nursing protocols for the correct management of these patients.

  We eliminated the ventilation phase of the standard protocol to prevent the spread of the virus. The scan begins by administering 99mTc-MAA in standard doses to acquire the perfusion phase. Next we perform the CT.

  We have followed the established Care Plan, checking the comfort and general condition of the patient at all times.

  Result Between March and June 2020, we performed 405 pulmonary SPECT-CT (203 women, 202 men; range 15-96 years) with high suspicion of PE. 311 patients (77%) were hospitalized and 94 (23%) were outpatients.

  Of the total, 72 patients (18%) attended without PCR, while the rest (333-82%) did undergo PCR, being positive in 67 cases (20%).

  In 137 cases (34%) SPECT-CT was positive for PET; of these, 56 (41%) were diagnosed with Covid+ with the previous CT.

  Conclusions The Hospital Germans Trias i Pujol is a reference center in the detection of T.E.P. in Catalonia.

  The management of the patient with Covid+ has hindered the work of Nursing in daily practice. By applying the appropriate protocol and offering precise nursing care, we have been able to perform the studies optimally with good diagnostic performance.
