Resumen de Abordaje “Degloving” del tercio medio de la cara en el tratamiento de tumores

  "Degloving" es un abordaje quirúrgico endobucal y endonasal del tercio medio de la cara, que permite una exposición amplia, simétrica y bilateral de, cavidad nasal, senos paranasales, sin producir cicatrices en piel. Complementando con osteotomías desarme óseo, se puede acceder a la base del cráneo y resecar tumores de esa localización. Describimos la técnica quirúrgica que fue utilizada en 56 pacientes del Hospital E. Rebagliati, portadores de tumores benignos y malignos. La endoscopía, TAC helicoidal y/o resonancia magnética, nos precisaron la localización exacta de los tumores que se distribuyeron en, cavidad nasal, senos paranasales, rinofaringe y base de cráneo. Los buenos resultados obtenidos tanto funcional como cosmético, evaluados por los cirujanos y por los pacientes, la incidencia baja de complicaciones y secuelas, nos permiten recomendar esta técnica quirúrgica como una opción terapéutica importante en el manejo de patología de localización en el tercio medio y base de cráneo.

  “Degloving” is an “endobucal” an “endonasal” surgical technique of the middle third of the face which allows an ample, aymmetrical and bilateral exposure of the nasal cavity, paranasal sinuses.Without making skinscars.This is complemented with osteotomies (bone derrangerment) so we can to reach the base of the skull and to resect tumours located in this region. We describe the surgical technic used in 56 patients admitted to the E.R hospital, because benign and malignant tumours of the head and neck. Endoscopy, helical ET and for MRI gave us the precise and exact location of these group of tumours distributed in the nasal cavity, paranasal sinuses, rhinopharinx and the skull base. The good results obtained functionally as well as cosmetically, were assessed by the attending surgeons and for the patients and their relatives involved The low incidence of sequelae and complications allow us to recommend this surgical technique as an important therapeutic option in the management of middle third and of the skull pathology.
