Síndromes de compresión nerviosa como manifestación de hipotiroidismo: reporte de caso

    El hipotiroidismo es el síndrome clínico caracterizado por la deficiencia de hormona tiroidea en el organismo. Este provoca cambios en el tejido conectivo, que se manifiestan como aumento de volumen y edema en distintas partes del cuerpo. Las alteraciones en el tejido conectivo pueden provocar síndromes de compresión nerviosa en el paciente hipotiroideo, siendo el síndrome de túnel carpiano el más frecuentemente asociado. En el presente artículo se relata el caso de un paciente que consulta por parestesias en miembro superior derecho y lumbalgia de características neuropáticas que llevaron al diagnóstico de síndrome del túnel carpal y neuropatía ciática, respectivamente. Se realizaron exámenes de laboratorio que documentaron hipotiroidismo severo, por lo que se inició tratamiento con levotiroxina. Seis semanas después, el paciente presenta resolución completa de las parestesias en miembro superior derecho y de la lumbalgia, lo que llevó finalmente al diagnóstico de síndromes de compresión nerviosa como manifestación del hipotiroidismo.

    Hypothyroidism is the clinical syndrome characterized by the deficiency of thyroid hormone in the body. It causes changes in the connective tissue, which manifests as an increase in volume and edema in different parts of the body. Changes in the connective tissue can cause nerve compression syndromes in hypothyroid patients, with the carpal tunnel syndrome as the most frequently associated. This article presents the case of a patient with paresthesias in the right upper limb and low back pain with neuropathic characteristics which led to the diagnosis of carpal tunnel syndrome and sciatic neuropathy, respectively. Laboratory tests documented severe hypothyroidism and levothyroxine was subsequently started. Six weeks later, the patient presents complete resolution of paresthesias of the right upper limb and low back pain, which finally led to the diagnosis peripheral mixta lumbar neuropathy as manifestation of hypothyroidism
