Hernia lumbar de Petit: A propósito de un caso

    La hernia lumbar de Petit es un defecto raro de la pared posterior abdominal en la que hay protrusión de la grasa retroperitoneal, del peritoneo o de un órgano intraperitoneal, que puede ser de origen congénito o adquirido, a nivel del triángulo lumbar inferior, limitado por la cresta del hueso ilíaco como base, el músculo oblicuo externo como borde lateral y el músculo dorsal ancho como borde medial. Presentamos el caso de un paciente de 45 años que acudió a consulta por dolor en región lumbarderecha. Al examen físico se palpa aumento de volumen en la región lumbar derecha (teórico triángulo de Petit) de 8 cm de diámetro máximo que se reduce manualmente y que protruye con la maniobra de Valsalva. Se decidió intervenir quirúrgicamente con el diagnóstico clínico de hernia lumbar de Petit. Se realiza hernioplastia retromuscular lumbar con malla de polipropileno. El postoperatorio del paciente transcurre sin incidencias.

    Petit's lumbar hernia is a rare defect of the posterior abdominal wall in which there is protrusion of retroperitoneal fat, peritoneum or an intraperitoneal organ, which may be of congenital or acquired origin, at the level of the lower lumbar triangle, limited by the crest of the iliac bone as the base, the external oblique muscle as the lateral border and the latissimus dorsi muscle as the medial border. We present the case of a 45 year old patient who consulted for pain in the right lumbar region. On physical examination, an increase in volume was palpated in the right lumbar region (theoretical Petit's triangle) with a maximum diameter of 8 cm that was manually reduced and that protruded with the Valsalva maneuver. It was decided to operate with the clinicaldiagnosis of Petit's lumbar hernia. Lumbar retromuscular hernioplasty with polypropylene mesh was performed. The patient's postoperative course was uneventful
