Resumen de Síndrome de sobreposición entre hepatitis autoinmune y cirrosis biliar primaria: reporte de caso

El síndrome de sobreposición se refiere a la coexistencia de dos patologías autoinmunes que comparten características clínicas, histológicas, serológicas y bioquímicas. En adultos se observa con mayor frecuencia la asociación de hepatitis autoinmune (HAI) y colangitis biliar primaria (CBP). Se presenta el caso de un varón de 54 años de edad, con un año de enfermedad, pérdida ponderal, ictericia y prurito. Su perfil hepático estuvo alterado, colestasis, anticuerpos antinucleares (ANA) positivo, anticuerpos antimitocondriales (AMA) negativo, serología viral negativa. La biopsia renal presentó hallazgos relacionados al síndrome de sobreposición HAI-CBP. Se concluye que el diagnóstico y tratamiento del síndrome de solapamiento de CBP-HAI es dificultoso y debe considerarse en el diagnóstico diferencial de las hepatopatías autoinmunes. Se recomienda la intervención multidisciplinaria para establecer el diagnóstico correcto y ofrecer estrategias terapéuticas favorables.

The overlap syndrome refers to the coexistence of two autoimmune pathologies that share clinical, his-tological, serological and biochemical characteristics. In adults, the association of autoimmune hepatitis (HAI) and primary biliary cholangitis (PBC) is observed more frequently. The case of a 54-year-old man is presented, with one year of illness, weight loss, jaundice and itching. His liver profile was altered, choles-tasis, positive ANA, negative AMA, negative viral serology. The renal biopsy presented findings related to the HAI-PBC overlap syndrome. It is concluded that the diagnosis and treatment of the overlap syndrome of PBC-HAI is difficult and should be considered in the differential diagnosis of autoimmune liver diseases. Multidisciplinary intervention is recommended to establish the correct diagnosis and offer favorable thera-peutic strategies.
