Hipertermia Maligna: Reporte de dos crisis en un paciente pediátrico

    Se presenta el caso de un niño de 33 meses de edad, previamente sano y sin antecedentes contributorios, quien ingresó a sala de operaciones para ser intervenido de orquidopexia. A noventa minutos de iniciada la cirugía, bajo anestesia general inhalatoria con sevoflurano, el niño presentó súbitamente hipercapnia, rigidez generalizada e hipertermia, debido a lo cual se brindó el tratamiento correspondiente, consistente en dantroleno intravenoso, manejo de la temperatura por medios físicos, ventilación mecánica invasiva convencional, sedoanalgesia y relajación e hidratación. Mejoró clínicamente en las primeras 24 horas, pero llamativamente presentó un segundo episodio 28 horas después del primero, que fue controlado rápidamente. Presentó una recuperación clínica y laboratorial a las 36 horas del primer episodio y fue dado de alta de la unidad de cuidados intensivos pediátricos al quinto día. Se presenta el caso por el cuadro inusual de recurrencia sin uso de anestésicos ni relajantes musculares despolarizantes y con el fin de motivar la necesidad de generar registros institucionales, regionales y nacionales de incidencia y prevalencia de esta enfermedad oculta y potencialmente mortal.

    The case of a 33-month-old boy, previously healthy and with no contributory history, is presented, who entered the operating room to undergo orchidopexy. 90 minutes into surgery, under inhalation general anesthesia with Sevoflurane, the child suddenly presented hypercapnia, generalized rigidity and hyperthermia, due to which the corresponding treatment was provided, consisting of intravenous Dantrolene, temperature management by physical means, conventional invasive mechanical ventilation, sedoanalgesia, and relaxation and hydration. He improved clinically in the first 24 hours, but strikingly presented a second episode 28 hours after the first, which was quickly controlled. He presented clinical and laboratory recovery 36 hours after the first episode and was discharged from the Pediatric Intensive Care Unit on the fifth day. The case is presented due to the unusual pattern of recurrence without the use of anesthetics or depolarizing muscle relaxants and in order to motivate the need to generate institutional, regional and national records of the incidence and prevalence of this hidden and potentially fatal disease.
