  El síndrome confusional agudo o delirium es una urgencia médica infradiagnosticada e infratratada en el entorno hospitalario. La sufren hasta un 30% de los pacientes ingresados por otras patologías médicas o quirúrgicas. Su no identificación y tratamiento implica un aumento de la mortalidad, hospitalizaciones prolongadas y exceso de incapacidad, sobre todo en ancianos frágiles.

  No existen fármacos que impidan su aparición, pero las medidas no farmacológicas y las intervenciones preventivas pueden reducir su incidencia y su gravedad. Pruebas tipo test como el CAM o el 4AT son fácilmente aplicables y podrían ayudar a identificar el delirium y minimizar su impacto.

  Por otro lado, es imprescindible diagnosticar la causa y tratarla, por lo que será necesario realizar una historia clínica detallada y las pruebas complementarias necesarias para el diagnóstico etiológico.

  Acute confusional syndrome or delirium is an underdiagnosed and undertreated medical emergency in the hospital setting. Up to 30% of patients admitted for other medical or surgical diseases develop it. Its lack of identification and treatment leads to an increase in mortality, prolonged hospitalizations, and excess disability, especially in frail older adults.

  There are no drugs to prevent its onset, but non-pharmacological measures and preventive interventions can reduce its incidence and severity. Multiple choice tests such as the CAM or the 4AT are easy to administer and can help identify delirium and minimize its impact.

  On the other hand, it is essential to diagnose the cause and treat it. Therefore, it will be necessary to take a detailed medical history and perform the necessary additional tests for the etiological diagnosis.
