Alteraciones topográficas en pacientes con astigmatismo corneal en consulta para intervención quirúrgica refractaria

    Introducción: los especialistas en topografía corneal incluyen varios mapas que describen la morfología y curvatura de la córnea. La técnica de aberrometría proporciona datos muy importantes al planificar las estrategias terapéuticas.Objetivo: determinar las alteraciones topográficas en pacientes con astigmatismo corneal, candidatos a intervención quirúrgica refractaria en el Centro Oftalmológico “Enrique Cabrera Cossío”.Métodos: se realizó un estudio transversal analítico a partir de una totalidad de 62 pacientes que cumplieron con los criterios del estudio y de ellos, 96 ojos posibles candidatos para intervención quirúrgica refractaria con láser excimer en el período de junio a noviembre de 2019. El estudio oftalmológico incluyó: antecedentes personales y oftalmológicos, examen del segmento anterior mediante la lámpara de hendidura Carl Zeiss, agudeza visual sin corrección con la cartilla de optotipo de Snellen, refracción dinámica y estática, dominancia ocular, microscopía endotelial, test de Schirmer, oftalmoscopia, test de sensibilidad al contraste, queratometría, tonometría, paquimetría y biometría. Se cumplieron los aspectos éticos.Resultados: 96,88 % con un patrón de astigmatismo corneal; en relación con los índices topográficos las irregularidades topográficas en 12,50 % y 11,50 % de excentricidad corneal. Hallazgo de dos pacientes con sospecha de queratocono y uno con degeneración marginal pelúcida.Conclusión: el patrón de astigmatismo corneal estuvo presente en la mayoría de las pacientes, con evidencias de alteraciones en los patrones e índices topográficos que demostraron la existencia de queratótomo y degeneración marginal pelúcida e irregularidades corneales, que afectan la calidad visual en córneas con otros parámetros aceptables para realizar la intervención quirúrgica refractaria.

    Introduction: specialists in corneal topography include several maps that describe the morphology and curvature of the cornea. The aberrometry technique provides very important data when planning therapeutic strategies.Objective: to determine the topographic alterations in patients with corneal astigmatism, candidates for refractory surgical intervention at the "Enrique Cabrera Cossío" Ophthalmological Center.Methods: an analytical cross-sectional study was carried out from a total of 62 patients who met the study criteria and of these, 96 possible candidate eyes for refractory surgery with excimer laser from June to November 2019. The ophthalmological study included: personal and ophthalmological history, examination of the anterior segment using the Carl Zeiss slit lamp, visual acuity without correction with the Snellen optotype chart, dynamic and static refraction, ocular dominance, endothelial microscopy, Schirmer's test, ophthalmoscopy, contrast sensitivity test, keratometry, tonometry, pachymetry and biometry. Ethical aspects were met.Results: 96.88% with a pattern of corneal astigmatism; In relation to the topographic indices, the topographic irregularities in 12.50% and 11.50% of corneal eccentricity. Finding of two patients with suspected keratoconus and one with pellucid marginal degeneration.Conclusion: the corneal astigmatism pattern was present in most of the patients, with evidence of alterations in the patterns and topographic indices that demonstrated the existence of keratome and pellucid marginal degeneration and corneal irregularities, that affect visual quality in corneas with other acceptable parameters to perform refractory surgical intervention
