Resumen de Tratamiento ortodóncico-quirúrgico de incisivo central superior impactado, asociado a diente supernumerario: reporte de caso

  La impactación de incisivos centrales superiores es un fenómeno raro que afecta la apariencia facial de un paciente pediátrico o adolescente con implicación estética y funcional, siendo los incisivos centrales de vital importancia para sonrisa, soporte del labio e incisión de alimentos. Las causas de impactación pueden ser consecuencia de traumatismos recibidos en edades tempranas, u obstrucciones físicas entre las que se reportan odontomas, mesiodens, dientes supernumerarios. Reporte de caso: Paciente femenino de 11 años de edad con maloclusión Clase I, con apiñamiento superior e inferior, biprotrusión y mordida cruzada del incisivo lateral superior derecho. El incisivo central superior derecho, presentó un retraso en su erupción de tres años, asociándose su impactación a un diente supernumerario y un fuerte desplazamiento palatino del incisivo lateral superior derecho, en mordida cruzada anterior. Se indicó ortodoncia correctiva, con aparatología fi ja de Straight Wire 0.22 de prescripción de Roth, la cual requirió de extracción de primeros premolares superiores e inferiores, para solucionar el apiñamiento y corregir la biprotrusión. Además, se removió el diente supernumerario, descubriendo quirúrgicamente el incisivo central superior impactado y traccionándolo hacia el arco. Se finalizó el caso en Clase I molar y canina, corrigiéndose la mordida cruzada anterior del incisivo lateral superior derecho. El tratamiento tuvo una duración total de 22 meses. Al final de la ortodoncia, se realizó gingivectomía para mejorar la arquitectura gingival, permitiendo así, una solución integral del caso, que le devolvió su función y estética a la paciente.

  Upper central incisor impaction is a rare phenomenon that aff ects the facial appearance of a pediatric or adolescent patient with aesthetic and functional involvement, being central incisors of vital importance for the smile, lip support and food incision. The causes of impaction can be consequence of trauma received at an early age, or physical obstructions from odontomas, mesiodens or supernumerary teeth. Case report: 11-year-old female patient with a class I malocclusion, with upper and lower crowding, biprotrusion and palatal displacement of the upper right lateral incisor presenting anterior cross bite. The upper right central incisor showed a delay in its eruption of three years, it’s impaction being associated with a supernumerary tooth. Corrective orthodontics was indicated, with Roth prescription Straight Wire 0.22 fi xed appliances, which required extraction of upper and lower fi rst premolars, to solve crowding and correct biprotrusion. In addition, the supernumerary tooth was removed surgically, uncovering the impacted upper central incisor and pulling it towards the arch. The case was fi nalized in molar and canine Class I, correcting the anterior crossbite of the upper right lateral incisor. The treatment lasted a total of 22 months. At the end of the orthodontics, a gingivectomy was performed to improve the gingival architecture, thus allowing a comprehensive solution to the case, which returned the function and aesthetics to the patient.
