Resumen de ¿Es realmente eficaz el MARPE en adolescentes y adultos jóvenes?: revisión sistemática de la literatura

  Introducción: el aumento en la demanda de tratamiento ortodóncico en adolescentes y adultos jóvenes, en los que el crecimiento esta limitado, nos llevó a plantearnos esta revisión sistemática de la literatura. Los objetivos fueron comprobar si el MARPE es una alternativa en el tratamiento de las maloclusiones transversales, determinar el porcentaje de expansión dental, dentoalveolar y esquelética, analizar el protocolo de activación más idóneo y revisar la tasa y los factores determinantes del éxito clínico.

  Material y métodos: en enero de 2021, siguiendo la declaración PRISMA, se realizó una búsqueda manual y electrónica en las principales bases de datos, se estableció la pregunta PICO y para evaluar la calidad de los estudios incluidos se emplearon las escalas de Newcastle-Ottawa, manual Cochrane, y la declaración STROBE.

  Resultados: se identificaron un total de 149 artículos. Después de eliminar los duplicados y aplicar los criterios de inclusión y exclusión se incluyeron un total de 13 artículos.

  Conclusión: el MARPE es una opción de tratamiento eficaz, siendo la expansión sobretodo esquelética y dental. El protocolo de activación más idóneo es de 2 vueltas al día hasta la aparición del diastema interincisal. La tasa de éxito fue del 89,81%. Los factores que influyeron en dicho éxito fueron la edad de inicio del tratamiento, el estadio de maduración de la MPS, el anclaje bicortical, la ubicación del tornillo, la densidad ósea y la longitud del paladar

  Introduction: the increase in the demand for orthodontic treatment in adolescents and young adults, in whom growth is limited, led us to consider this systematic review of the literature. The objectives were to verify whether MARPE is an alternative in the treatment of transverse malocclusions, to determine the percentage of dental, dentoalveolar and skeletal expansion, to analyze the most suitable activation protocol, and to review the rate and determinants of clinical success.

  Material and methods: in January 2021, following the PRISMA statement, a manual and electronic search was carried out in the main databases, the PICO question was established and the Newcastle-Ottawa scales were used to assess the quality of the included studies. , Cochrane Handbook, and the STROBE statement.

  Results: a total of 149 articles were identified. After eliminating duplicates and applying the inclusion and exclusion criteria, a total of 13 articles were included.

  Conclusion: MARPE is an effective treatment option, mainly skeletal and dental expansion. The most suitable activation protocol is 2 rounds a day until the appearance of the interincisal diastema. The success rate was 89.81%. The factors that infl uenced this success were the age of initiation of the treatment, the stage of maturation of the MPS, the bicortical anchorage, the location of the screw, the bone density and the length of the palate
