The inferior alveolar nerve is one of the major branches of the mandibular division of the trigeminal nerve. Sometimes anatomical variations may occur such as a bifid mandibular nerve, or accessory retromolar foramen or double or accessory mental nerve that may have implications in many current dental treatments such as anesthetic infiltrations and especially surgical treatments such as periodontal, endodontic surgery and removal of impacted inferior third molars. Also we make reference to other situations such as the presence of nerve anastomosis and accessory innervations that may be important in the anesthetic infiltration of the inferior alveolar nerve. Good complementary exploration, through orthopanthomography and CT, provides useful information for an adequate diagnosis of these anatomical variations. They also allow for their detection in order to avoid therapeutical complications that may lead to inferior alveolar nerve damage.
