Elaboração de um protocolo para manejo da terapia nutricional parenteral em um hospital público no Sudoeste da Bahia

    Parenteral nutritional therapy (NPT) is indicated to patients when the oral or enteral pathways are unable for nutrition. The correct indication, prescription, administration and monitoring, performed by the Multiprofessional Team of Nutritional Therapy, generates benefits to the patient and reduces the risk of complications. The aim of this study was to develop an NPT protocol to systematize care and direct the multidisciplinary team in flows, conducts and procedures to reduce complications to hospitalized patients. This is an observational and descriptive study, conducted in the medical and surgical clinic sectors, intensive care units and emergency of a Public Hospital in southwestern Bahia, between May 2019 and February 2021. To elaborate the protocol, the search for scientific literature was performed through the Databases PubMed/Medline, Lilacs and Virtual Health Library, later, the selection and reading of the guidelines of the Ministry of Health, ANVISA, and the Brazilian, American, Canadian and European societies was made. The main nonconformities found in the institution are related to the lack of communication between professionals and conflict in their attributions, late initiation of parenteral nutrition, errors in the prescription and indication of access route, absence of laboratory monitoring, difficulties with the evolution and discontinuation of NPT. Deficiencies in the conduct ive of parenteral therapy were identified and supported the elaboration of a nPT management protocol, however, this is only an initial step that should be followed by the implementation in the hospital unit and the training of the entire team.

    A terapia nutricional parenteral (TNP) é indicada quando as vias oral ou enteral estão impossibilitadas para a nutrição. A correta indicação, prescrição, administração e monitorização é realizada pela Equipe Multiprofissional de Terapia Nutricional. O objetivo do estudo foi elaborar um protocolo de TNP para sistematizar o cuidado e direcionar a equipe multiprofissional em fluxos e condutas em prol de redução de complicações. Trata-se de estudo observacional e descritivo, realizado nas clínicas médica e cirúrgica, unidades de terapia intensiva e emergência de um Hospital Público do Sudoeste da Bahia, entre maio de 2019 a fevereiro de 2021. Para elaboração do protocolo, foi realizada a busca de literatura científica, através das bases de dados PubMed/Medline, Lilacs e Biblioteca Virtual em Saúde. As principais inconformidades encontradas na instituição estão relacionadas à falta de comunicação entre os profissionais e conflito nas suas atribuições, erros na prescrição e indicação da via de acesso, ausência de monitoramento laboratorial, dificuldades com a evolução e descontinuação da TNP. As deficiências identificadas na condução da TNP, deram suporte a elaboração de um protocolo de manejo de TNP, entretanto, esta é apenas uma etapa inicial que deve ser seguida pela implantação na unidade hospitalar e treinamento de toda equipe.
