Resumen de Tuberculoma de fosa posterior: una presentación inusual de tuberculosis extrapulmonar

  Un tuberculoma es un conglomerado de focos de necrosis caseosa en el parénquima cerebral que se produce por diseminación hematógena La tuberculosis del sistema nervioso central presenta una incidencia de 0.06 por millón de habitantes, correspondiendo al 1% de todos los casos de tuberculosis y de 6-10% de las formas extrapulmonares en pacientes inmunocompetentes. Presentamos el caso de un paciente de 57 años con cefalea de 48 horas de evolución, holocraneana, prosopagnosia y agitación psicomotriz con tendencia a la violencia. Concomitantemente refiere fiebre no cuantificada de predominio nocturno, de igual evolución. Se realiza hemograma con hallazgos normales, serología: VHB, VHC, VIH negativos. Rx de tórax que muestra engrosamiento pleural. TAC e IRM que reportan lesión con patrón de realce en anillo en cerebelo. Se realiza punción lumbar reportando: proteínas: 456mg/dL, glucosa: 10mg/dL, células: 105mm3, mononucleares: 100%, ADA: 23.84U/L, GeneXpert: positivo. Se brinda tratamiento antifímico y esteroides. Paciente evolucionó satisfactoriamente.

  A tuberculoma is a conglomerate of foci of caseous necrosis in the brain parenchyma produced by hematogenous dissemination. Tuberculosis of the central nervous system has an incidence of 0.06 per million inhabitants, corresponding to 1% of all cases of tuberculosis and 6-10% of extrapulmonary forms in immunocompetent patients. We present the case of a 57-year-old patient with headache of 48 hours of evolution, holocranial, prosopagnosia and psychomotor agitation with tendency to violence. At the same time, he reported unquantified fever, predominantly at night, with the same evolution. CBC with normal findings, serology: HBV, HCV, HIV negative. Chest X-ray showed pleural thickening. CT and MRI showed lesion with ring enhancement pattern in cerebellum. Lumbar puncture was performed reporting: protein: 456mg/dL, glucose: 10mg/dL, cells: 105mm3, mononuclear: 100%, ADA: 23.84U/L, GeneXpert: positive. Antipyretic treatment and steroids were given. Patient evolved satisfactorily.
