El origen anómalo de la arteria coronaria izquierda del tronco de la arteria pulmonar o Síndrome de Bland-White- Garland es una anomalía congénita rara. Constituye una de las causas más comunes de isquemia miocárdica e infarto en niños y si no se le trata, el 90 % de los pacientes muere sin alcanzar el año de vida. Es por esto que se presenta el caso de una lactante de 6 meses de edad con este diagnóstico, a la que se le realizó la corrección quirúrgica, mediante elongación de la arteria coronaria izquierda con parte de la pared de la arteria pulmonar y aorta; una opción quirúrgica efectiva cuando las condiciones anatómicas impiden realizar alguna de las técnicas habituales. La paciente evolucionó favorablemente; la insuficiencia mitral evolucionó de grave a ligera-moderada, sin tratamiento directo sobre la válvula. La principal complicación cardiovascular fue la disfunción de ventrículo izquierdo, que fue mejorando de forma progresiva.
The anomalous origin of the left coronary artery from the trunk of the pulmonary artery or Bland-White-Garland syndrome is a rare congenital anomaly. It is one of the most common causes of myocardial ischemia and infarction in children and if it is not treated, 90 % of patients die before reaching the age of one year. This is why we report the case of a 6-month-old infant with this diagnosis, who underwent surgical correction by elongating the left coronary artery with part of the wall of the pulmonary artery and the aorta; an effective surgical option when anatomical conditions prevent performing any of the usual techniques. The patient evolved favorably and the mitral regurgitation progressed from severe to mild-moderate, without direct treatment on the valve. The main cardiovascular complication was left ventricular dysfunction, which progressively improved.
