El espectro de infecciones que afectan el cerebro neonatal se ha ampliado en los últimos años al incluir, además del tradicional síndrome TORCHS (toxoplasmosis, rubéola, citomegalovirus, herpes, sífilis), otros agentes infecciosos como el virus de inmunodeficiencia humana, la varicela, los enterovirus, el virus de la coriomeningitis linfocítica humana y otros agentes virales neurotróficos. El objetivo de la presente revisión es profundizar en los mecanismos que generan compromiso del sistema nervioso central en este tipo de infecciones y describir sus manifestaciones clínicas con énfasis en los signos y síntomas neurológicos. Un alto porcentaje de infecciones no se diagnostican al nacer y las manifestaciones clínicas se hacen evidentes en etapas posteriores. La detección y tratamiento tempranos dependen del diagnóstico prenatal y de la sospecha clínica y permiten hacer intervenciones que minimicen las secuelas neurológicas a largo plazo.

The spectrum of central nervous system congenital infections has expanded in the last years, moving from the traditional TORCHS syndrome (toxoplasmosis, rubella, citomegalovirus, herpes simplex, syphilis) towards a wide variety of infectious agents that include human immunodeficiency virus, varicella virus, enteroviruses, lymphocytic choriomeningitis virus and other neurotropic viruses. The aim of this review is to deepen into the mechanisms of central nervous system involvement in congenital infections and to describe the clinical manifestations with emphasis in neurological signs and symptoms. A high percentage of these infections remain undiagnosed in the newborn and become evident at late infancy or preschool age. Early detection and treatment depend on prenatal screening and high clinical suspicion and permit interventions that minimize long term neurological sequelae.
