      La enfermedad diverticular es la anomalía morfológica más frecuente del colon cuya prevalencia se ha incrementado en las últimas décadas, generando importantes costos a nivel mundial. Objetivo: analizar los resultados del manejo conservador de la diverticulitis aguda complicada. Materiales y métodos: se trata de un estudio de revisión no sistemática, de tipo descriptivo. La revisión se llevó a cabo mediante consulta a las bases de datos y motores de búsqueda: Medline, Cochrane, Medigraphic, Embase, Dialnet, PubMed, SciELO, LILACS, Elsevier, Scopus y Scholar academic. Se han incluido artículos originales, artículos de revisión, revisión sistemática y metaanálisis, publicados en el periodo 2019 – 2024, en idiomas español e inglés, disponibles a texto completo. Resultados: la tasa de éxito del manejo conservador de la diverticulitis aguda ha sido documentada entre 83,6% - 97,5%, mientras que la recurrencia suele presentarse entre el 16,1% - 28,1%. Conclusión: en función de la evidencia científica disponible en la actualidad es posible concluir que el manejo conservador de la diverticulitis aguda complicada ofrece resultados favorables, con adecuada tasa de éxito y reducida tasa de recidiva, al tiempo que permite reducir la necesidad de efectuar ostomías. Por tanto, se puede afirmar que se trata de una opción terapéutica válida y eficaz, siendo prioritaria la adecuada selección de los pacientes que serán manejados de forma conservadora, puesto que los registros clínicos disponibles revelan que una parte de estos, progresará a cuadro séptico severo, el cual incrementa la morbilidad y mortalidad.Diverticular disease is the most common morphological abnormality of the colon, the prevalence of which has increased in recent decades, generating significant costs worldwide. Objective: To analyze the conservative management outcomes of complicated acute diverticulitis. Materials and methods: This is a non-systematic, descriptive review study. The review was carried out by consulting the databases and search engines: Medline, Cochrane, Medigraphic, Embase, Dialnet, PubMed, SciELO, LILACS, Elsevier, Scopus and Scholar academic. Original articles, review articles, systematic reviews and meta-analyses, published in the period 2019 - 2024, in Spanish and English, available in full text, have been included. Results: The success rate of conservative management of acute diverticulitis has been documented between 83.6% - 97.5%, while recurrence usually occurs between 16.1% - 28.1%. Conclusion: based on the scientific evidence currently available, it is possible to conclude that conservative management of complicated acute diverticulitis offers favorable results, with an adequate success rate and a reduced recurrence rate, while reducing the need for ostomies. Therefore, it can be stated that this is a valid and effective therapeutic option, with the proper selection of patients who will be managed conservatively being a priority, since the available clinical records reveal that a proportion of these will progress to severe septic symptoms, which increases morbidity and mortality.Recibido: 12-06-2024Aprobado: 18-07-2024Publicado: 01-11-2024
